Physical examinations and up to date immunizations are required for all children prior to attendance at ECC. Due to state licensing requirements, the physical form provided in the registration packet, Child Care Center Health Record, State Form 49969, is the ONLY acceptable documentation of a physical. The form must be completed and signed by the child’s physician. The physical (well child exam) must have been completed within 1 year prior to the start of the school year. If a child has a special diet, allergies, asthma, seizure disorder or other health condition, a Health Care Plan must be completed and signed by the child’s doctor (see school nurse for specific forms). If a child has a milk/dairy allergy a doctor’s note will be required, it must also state if the child may eat other food items with milk as an ingredient and if almond or soy milk should be substituted. A Community Health Network Consent to Treat and HIPPA Authorization Form must be completed annually for all children. This form is included in ECC’s registration packet. While attendance is important, please do not send your child to preschool/childcare if they are ill or cannot participate in typical activities. ECC requests that all absences (and the reason for the absence) be reported to the front office,317-869-4750, by 10:00 a.m. the morning of the illness (see Illness Policy). All meals/snacks are provided. No meals, snacks or drinks can bebrought in to ECC unless a doctor completes the Eating and Feeding Evaluation Form and our center cannot meet a child’s specific dietary needs (see Food Policy). This only applies to medical conditions that require specific food or drink items. The giving or application of all medication and carrying out of medical procedures shall be done only on written order from a physician. This includes; chapstick, hand lotions and all over the counter medications (see Medication Policy).

Healthy Smiles Mobile Dentist will visit the Warren Early Childhood Center in both the fall and the spring. More detailed information and permission forms will be sent home prior to their visit(s).
